FORMS

Property Line Easement Form - Download form here.

  • Adjacent Owner must consent to the drilling of a well that is to be located closer than 100 yards (300 feet) from the property line

  • Does not apply if applicant owns the adjacent property

  • Does not apply if replacing an existing well

Agent Authorization Form - Download form here.

  • Must be signed by property owner

  • Gives agent the authority to manage test holes, wells, and meter production reporting

  • Owners must have one on file for drillers to take out permits

Abandonment Form - Download form here.

  • All abandoned wells should be filed with the District

  • If abandonment form is not on file for a well, then spacing requirements are in effect as though that well exists

Reclassification Form - Download form here.

  • Lowers or raises the maximum production of a well

  • Must be filed if the new well does not meet current spacing requirements with an established well that was classified at a higher production rate than what it is capable of producing

  • Must be filed before the new well is to be drilled

  • A reclassified well will be checked for the GPM
EXAMPLE:  New well is classified as a class C but is only 400 yards from a class
                          D.  The class D actually produces only 600 GPM, therefore if it was
reclassified as a Class C, the spacing requirements would be met.
